How do I make an appointment?

Patients at Lake Placid Sports Medicine are seen by appointment. Appointments may be made for follow-up visits or new orthopaedic problems you may be experiencing.

What provider should I see?

Drs. Bullock, Byrne, Szczech, and Smith are all Board Certified orthopaedic surgeons.  Dr. Viola is a Board Certified family physician with a Certificate of Added Qualifications in Sports Medicine.  All are dedicated to providing exceptional care to every patient.  When making your appointment, please describe your situation and present any special circumstances or requests. The receptionist will direct you to the most appropriate provider for your situation.

How do I get to the office?

Lake Placid Sports Medicine is located within Adirondack Medical Center Lake Placid at 203 Old Military Road, Lake Placid, NY. Enter the building through the main front doors. The Lake Placid Sports Medicine reception windows are across the entrance to the left.

What do I need to bring to my appointment?

We ask that both new and returning patients bring the following to their appointments:

  • Any of the forms that you completed at home
  • Your insurance card
  • A photo ID
  • Insurance referral, if required
  • Your co-payment. We gladly accept cash, checks, money orders, MasterCard, and Visa.
  • Any X-rays, medical records, lab results, or other pertinent medical history, including visits to other medical specialists and any previous surgeries
  • A list of current medications

Is pre-authorization needed?

To determine whether pre-authorization is needed to see a physician at Lake Placid Sports Medicine, please contact your insurance company. If required, authorization must be in place at the time of the first appointment.

Lake Placid Sports Medicine participates with many insurance companies, including the following:
